Pointe Coupee Homebound Health Services offers a comprehensive range of services including Nursing Care, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker services, and Home Health Aide services. We cater to patients with diverse illnesses and conditions.

  • Wound care including management of negative pressure devices

  • IV therapy

  • Advanced Illness Management Program

  • Epidural pain pump management

  • Chronic and acute disease management for diabetes, CHF, COPD, Alzheimer’s disease, Parkinson’s disease and others

  • Monitoring of PT/INR in the home for patients on “blood thinners”

  • After surgery care: including use of specific joint replacement protocols and heart surgery protocols

  • Medication management, injection administration teaching

  • Low vision education

  • Vestibular rehabilitation

YOU MAY NEED HOME HEALTH CARE IF YOU ARE CONSIDERED HOMEBOUND AND YOU ARE:

 
 
  • Frequently hospitalized or visiting the emergency room

  • Taking multiple medications with frequent medication changes

  • Forgetting to take medications and/or injections or need instructions on administering your own injections

  • Having a decline in personal hygiene, including trouble dressing or bathing

  • Recently home from the hospital or nursing home

  • Experiencing a notable weight loss

  • Recently diagnosed with a new condition or illness

  • Experiencing unstable vital signs, such as an uncontrolled blood pressure

Hospice Services

Loving care at home when you need it most.

Pointe Coupee Hospice has been in operation since 2001 and offers our most experienced nursing team for pain management and symptom control. Hospice offers a team approach to manage a patient’s medical, emotional, and spiritual needs. Care is patient and family centered to meet individual needs. Our hospice team believes that every person has a right to dignified and pain-free end-of-life. We work closely with our hospice physician, Dr. Ashley Bordelon, as well as your primary care or specialty physician to manage symptoms. Medical social work, chaplain, and volunteer services are hospice care components that help to support patients and families emotionally, spiritually, and socially. Hospice care can also include many of the other home health services that you are familiar with such as physical, speech & occupational therapy.

Hospice care is most often provided where the patient lives, whether in their own home, the home of a family member, a nursing home or assisted living facility. Inpatient hospice care in the hospital can also be provided if most appropriate.

Our team provides comforting and compassionate care to patients and their families with a goal of enhancing quality of life at a time when every moment shared and memory made is most precious.

Pointe Coupee Nutrition Program

A healthier you, day by day.

People who eat a healthy diet live longer and are at lower risk for serious health problems such as heart disease, type 2 diabetes, and obesity. For people with chronic diseases, healthy eating can help manage these conditions and prevent complications.

The Pointe Coupee Nutrition Program offers personalized education and training tailored to the patient's needs and physician's recommendations. This program is provided at no cost to participants. We have a Registered and Licensed Nutritionist, as well as a Certified Diabetes Educator, available by appointment. Call (225) 638-5717 to schedule!
