"Five Wishes" Workshop
Pointe Coupee Homebound Health and Hospice will be hosting a free “Five Wishes” workshop. PCHHH is hosting this workshop because we understand planning for the future involves more than pension benefits or life and health insurance. We realize many of you are facing important decisions about medical care, especially with aging or ill family members, and that is why we wanted to make “Five Wishes” available to you.
“Five Wishes” is a document developed by a nonprofit organization called “Aging with Dignity” designed to help you and your family discuss, decide and describe the kind of care and support you wish in the event of serious illness. PC Home Health & Hospice is proud to be at the forefront of the movement to bring advance care planning to the community and we hope that you find this service useful now and in the years ahead.
Participants will be given a free “Five Wishes” booklet that becomes your legal advanced directive that is recognized by the state of Louisiana. Participants will be shown an information DVD on how to fill out the “Five Wishes” document. The seminar will be facilitated by members of the PC Hospice Team.
